We work in accordance of General Data Protection Regulation (GDPR) and therefore all your data is private and treated with the correct security and confidentiality. Your data is held for the purposes of veterinary physiotherapy treatment and follow up services. Your data consists of your name, address, email address, telephone number, mobile phone number, insurance details and any details from your veterinarian. We may receive the above information from paraprofessionals like veterinarians and insurers where this information is essential for FP Animal Therapy Centre Ltd to perform their veterinary physiotherapy services.
Veterinary physiotherapists work under vet referral by law and therefore share notes on the animal with the referring vet practice. We may also share personal data with other paraprofessionals, however this will only be shared through your given consent. Data will never be shared without your prior consent unless FP Animal Therapy Centre Ltd are under legal obligation to comply to sharing of some information on the animals medical record/notes.
How long do we hold your personal information?
We are lawfully required to keep veterinary physiotherapy records and treatment notes for 8 years from last treatment, these can be paper or electronic based. Both electronic and paper based records are stored securely and can only be accessed by authorised staff members. We store this information for our own use only. Your accounts can be kept for up to 7 years and once again are securely stored.
Once this time has lapsed this information and records kept are shredded or disposed of securely.
Cookies
A cookie is used to store small amounts of information. This information is collected for traffic analysis only. The cookie does not contain personal details. You can choose to refuse/accept or block cookies, if you choose to block them however, it may prevent you from taking full advantage of the website.

Accessing your data
If you would like to access your personal data then formal identification will need to be shown. This can be through the use of passport, driving license and a utility bill not older than three months. Please contact FP Animal Therapy Centre Ltd via the website to request this.

In the event you would like to make a complaint about how your data is being processed then please contact via the website or email (flora@fpatc.co.uk). If this complaint is not dealt with within 30 days then you have the right to contact the ICO.
